For printable forms:

4. Attach it to an email and send to office@st-katherine.org or bring it to the church office.

5. To ensure legibility, it’s preferred that you fill in the form on-line then print/submit or save and email it. If you choose to print the blank form and hand-write the names, please write legibly.

   FOR A GIVEN FAST/FEAST PERIOD, PLEASE DO NOT RE-SUBMIT LIVING/DEPARTED NAMES FOR EACH SERVICE. THE LISTS ARE RETAINED AND READ AT EACH SERVICE.
